First time enrollment fee $100 per adult over the age of 18 yrs.

Cancel your membership anytime.

If you choose to discontinue membership at any point, you are eligible to re-enroll one time only for $200 per adult.

*Membership does NOT constitute insurance. Black Olive recommends all uninsured patients obtain a minimum of catastrophic coverage.

ACUTE CARE

Treatment of simple conditions such as UTI, URI, cough, allergies, sinus infections, bronchitis, ear/eye infections, minor skin rashes/conditions, mild stomach virus can be provided through Telehealth.

CHRONIC CARE

Anemia, Asthma, COPD, Diabetes, GERD, Hypertension, Hypothyroidism, Hyperlipidemia, Obesity. Initial visit in office and follow up visits either in person or via Telehealth
